1.Required Information for Application:
- Applicant’s name, date of birth, telephone number, residence (or address), National ID number, and email address. If the applicant is a legal entity or other organization with a manager or representative, provide the organization’s name, office or business address, and the manager’s or representative’s name, date of birth, telephone number, residence (or address), and email address. (required)
- If the applicant has an agent, provide the agent’s name, date of birth, telephone number, residence (or address), National ID number, and email address. If the agent is appointed, submit a power of attorney. If the agent is statutory, state the relationship and attach copies of relevant supporting documents. (Please send us the following documents by email.)

2.Alternative Application Method:
The applicant may also complete this Branch’s file access application form(signed in person), scan the application form and attach it as electronic file.
3.Notification for Corrections:
After submitting the application, if any corrections are required, this Branch will promptly notify the applicant in writing and by email to make the corrections within seven days.
4.Notification of Application Result:
This Branch will notify the applicant of the approval or rejection result in writing and by email no later than 30 days from the date of receipt or correction.
